Pet Owner Information & Representation

I (Owner) represent that I am the legal owner of the named dog(s) on my account and I assume all risks, dangers, and responsibility for injuries to my pet(s). Owner understands and agrees that Owner is solely responsible for any harm while Owner’s dog(s) is/are attending Daycare / Boarding / Self Wash / Training.

Acknowledgement of Risks and Responsibilities:

I, the undersigned, recognize that there is an inherent risk or injury in any environment associated with cageless/multiple dogs. I also recognize that such risks include, without limitation, injuries or illnesses resulting from fights, rough play and contagious diseases. Knowing these inherent risks and dangers, I warrant that I will abide by all safety rules and instructions. I agree by this contract to assume full responsibility and hold MANGO AND MINNOW’S DOGGY DREAMLAND INC, its agents or employees harmless for said pet(s) illness, bodily injury, death or other damage as a result of any incident including my or others negligence, except to the extent that damage or injury may be due to willful misconduct. I further agree to hold MANGO AND MINNOW’S DOGGY DREAMLAND INC harmless and indemnify it against all defense costs, fees and business losses resulting from any claim I may make or cause to be made against MANGO AND MINNOW’S DOGGY DREAMLAND INC, for which it, its agents or employees are not ultimately held to be legally responsible.

Vaccination & Health Requirements

• Requirements: All pets must have current vaccinations (Rabies, Parvo, Distemper, Bordetella) and be on flea/tick prevention. Proof of vaccination is required before admission and upon expiry date on file.
• Spay/Neuter: All pets must be spayed or neutered by 6 months of age.
• Health Policy: Owner is not enrolling any dog’(s) that has any condition that could potentially jeopardize the health of other dogs or people and has not had any potentially communicable condition within 30 days prior to enrollment. Owner further understands that even if Owner(s) dog’(s) is vaccinated for Bordetella or other airborne dog-dog transmitted ailments (i.e Kennel Cough) there is a chance that the Owner’s dog’(s) can still contract said ailments. Owner agrees that Owner will not hold MANGO AND MINNOW’S DOGGY DREAMLAND INC
  responsible if Owner’s dog(s) contracts any dog-dog transmitted ailments.
• Veterinary Liability and Care: Owner agrees to allow MANGO AND MINNOW’S DOGGY DREAMLAND INC to obtain medical treatment for Owner’s dog’(s) if they appear ill, injured, or exhibit any other behavior that would reasonably suggest that they may need medical treatment including anesthesia. Owner agrees to be fully responsible for the cost of any such medical treatment. Owner gives permission to MANGO AND MINNOW’S DOGGY DREAMLAND INC to use Nimbus Pet Hospital within their working hours or seek emergency veterinary care at the closest available clinic.
• Weakened Immune System: Owner understands that special-needs dogs, puppies, dogs that have had past injuries/major surgeries, and senior dogs naturally have a higher risk of injury, stress-related illnesses, weakened immune systems or exacerbation of any pre- existing conditions. As such, by using our facility for daycare or boarding the Owner is waiving any claim for injury or illness experienced by Owners dog(s) while in our care.

Behavioral & Safety Policies

• Temperament: Pets must be sociable. We reserve the right to refuse service or dismiss any pet that displays aggressive or unruly behavior.
• Group Play: All dogs must pass a general behavior assessment in order to attend doggy daycare and/or boarding. No dog may be admitted to our facility who shows any signs of any type of aggression. Owner further understands and agrees that in admitting Owner’s dog(s) to doggy daycare and/or boarding, Owner is representing to MANGO AND MINNOW’S DOGGY DREAMLAND INC that Owner’s dog(s) is/are in good health and have not harmed, shown aggression, or exhibited any threatening behavior towards any person or other dogs. Any dog demonstrating aggression or other behavior deemed unacceptable by our staff may be required to leave daycare/boarding.
• Group Play Risks: While staff supervises all dogs, owner acknowledges that interacting with other dogs carries risks of minor scratches or injuries. If your dog is showing signs of overstimulation or physical overexertion, we reserve the right to give them a break in a safe, enclosed environment away from other pets. This should not exceed more than 20 minutes consecutively.
• Leash Law: Please be considerate and comply with the Oregon Leash Law. We kindly ask that ALL pets must enter and exit on a leash or in a carrier. If we are taking your pet outside of the facility our staff will always have a secondary slip lead on your pet in conjunction with the proper collar/harness and leash your pet comes with.

Operating Hours, Booking, & Cancellations

• Daycare Hours are from 6:30am- 12pm then 1pm-6:30pm.
• Boarding Check-in from 1pm-5pm. *Early drop-off can be accommodated but there will be an additional charge of a half day of daycare.
• Boarding Check-out is from 8am-11am. *If your dog is not picked up between these times you will be charged for additional daycare or overnight services depending on pick up
  date/time.
• Late Pickup: Dogs not picked up by closing time will be boarded overnight, with boarding fees applied. (If occupancy allows)
• Cancellations: We ask for a 48 hour notice of any cancellation or modifications to your pets visit or scheduled stay. Less than a 48 hour notice could result in a forfeit of your deposits or prepaid visits for the dates that were scheduled.
• Food/Meds: When boarding overnight, Owner agrees that it is the Owner’s responsibility to leave an adequate supply of food and medications for Owner’s dog(s) during the entire time Owner’s dog is cared for by MANGO AND MINNOW’S DOGGY DREAMLAND INC. We ask that medications come in their original, labeled prescription bottles and that food be in a labeled ziplock type bag or an appropriate sized sealed container for the length of their stay. *We do not have space for large containers so please be considerate of the amount of food you give us.

Fees & Payments

• Payments are due at the time of service. We accept cash, debit, and credit payments.
• Package Expiration: Prepaid daycare packages expire 90 days from date of purchase and are non-refundable.

Liability Waiver & Media Release

• Personal Property: Owner understands that Owner is solely responsible for any harm, including to any other pet(s), to the employees or invitees of MANGO AND MINNOW’S DOGGY DREAMLAND INC, or to the equipment, facilities, or other property of the facility, caused by Owners dog(s). Owner also agrees that MANGO AND MINNOW’S DOGGY DREAMLAND INC shall not be responsible or liable for any lost or damaged personal property belonging either to Owner or Owner’s pet.
• At Mango and Minnow’s we love to take pictures/videos of your pets for things such as profile pictures, progress reports, social media/advertising, etc. By signing you allow the facility to use your pets photo/videos for the above. Live camera footage will be viewable to the public.

Right Of Refusal

• MANGO AND MINNOW’S DOGGY DREAMLAND INC reserves the right to refuse admittance to any dog’(s) or dismiss any dog that does not meet or maintain the health, temperament or other daycare/boarding standards. The determination shall be made at the sole discretion of MANGO AND MINNOW’S DOGGY DREAMLAND INC.

Owner hereby certifies that Owner has read and understand this Waiver and Release of Liability and the regulations set forth above. By signing this agreement, Owner agrees to be bound by its terms and conditions.
